PAIA Manual
This manual is compiled in terms of section 51 of the Promotion of Access to Information Act 2 of 2000 (PAIA), as amended by the Protection of Personal Information Act 4 of 2013 (POPIA). Every private body in South Africa is required to make this manual available at its principal place of business and on its website.

2. Guide on how to use PAIA
The South African Human Rights Commission has compiled a guide on how to use PAIA, as required by section 10 of the Act. This guide is available from the South African Human Rights Commission.

3.2 Records held that may be requested

3.3 Records not held
MS Nova Meditech Pty Ltd does not hold the following categories of records, for any Companion app:
- Users' health readings, symptom logs, medication records, appointment records, self-check scores, or vault documents — these remain device-local only and are never stored on our servers.
- Users' physical addresses, ID numbers, or dates of birth.
- Card or bank details (payment processing is handled entirely by our payment services provider, where a paid tier exists).
- Marketing or advertising data.
4. How to submit a PAIA access request
A request for access to records must be made in writing. A prescribed PAIA request form (Form C) is available from the South African Human Rights Commission website. You may also submit a written request by email.

Your request must include:
- Your full name and contact details.
- A description of the record(s) you seek access to, including which Companion app it relates to (if applicable).
- The form in which access is requested (copy, inspection, etc.).
- If you are requesting on behalf of another person, proof of that person's authorisation.
We will respond within 30 days of receiving your request, as required by PAIA section 56. We may request a deposit as permitted by the PAIA regulations. Access may be refused on the grounds set out in Part 3 of PAIA (sections 62–69).
5. Grounds for refusal of access
Access to records may be refused on the grounds set out in sections 62–69 of PAIA, which include (among others):
- Records containing personal information of a third party (unless that party consents or the requester is the data subject).
- Records that are commercially confidential.
- Records that would prejudice research or the safety of individuals.
6. Information Regulator
If your PAIA access request is refused, you may lodge an internal appeal (section 74) or approach the Information Regulator (section 77) or a court. The Information Regulator's contact details:

This manual is available on this website at myflaresa.co.za/paia/, and on request by email at admin@vaxtrack.co.za. It will be updated as required, including as new Companion apps join the family. The "Last updated" date below reflects when it was last revised.
